[nsp] unsupported PA in VXR

We had this happen with a PA-A3-OC3MM today in a 7206VXR/400/2FE, the card
is not recognized in the slot 2 but works fine in slot 6 (same PCI bus). We
reinserted the card multiple times and still came up with the error. Defect
slot?
