[nsp] ATA-186's back to back?

If you want a quick and easy solution, use a public SIP proxy server such as
http://fwd.pulver.com or http://iptel.org  It's essentially the same thing
as Vonage minus the PSTN gateway.

This is an especially nice if you don't have static ip's available to you.
Setting up your own sip proxy isn't that bad either if you don't mind *nix
(vovida and iptel's sip serversare open source).
