[nsp] IP-translation; Destination NAT; Proxy Enforcement. Alternatives?

hi,

which solution alternatives do I have for realizing the following
task:

I want a cisco router to change the destination IP-address in packets,
which originate from specific hosts and want to use a specific
service (tcp/udp port).

Source_IP, Source_Port, Destination_IP, 80
--> Source_IP, Source_Port, New_Destination_IP, 80.

Maybe there is a way to realize source specific routing
"All Packets coming from $Source_IP, don't route to the default
route, but to my proxy as next hop."
