The only good NMS is the one you write yourself.

If every network was the same I'm sure we'd have a perfect NMS solution
available, but unfortunately they are not, whether it be a commercial
solution or open source, someone still has to write some "glue" to tie
them together, some of the things you'd expect from an NMS for a service
provider:

* Data logging and analysis of various metrics
* Configuration management across all various types of hardware
* Disaster recovery 
* Capacity planning
* ACL's and permissions to manage who can change / see what.
* Customer tracking of lodged faults & issue's
* Reporting 
* Integration with a help-desk ticket tracking system

And the list goes on..

Current job I have has a setup with many different types of software to
cater for each task which is a PITA, but we don't have millions to blow
on NMS either. We run a mixture of rancid, cacti, nagios to achieve
above.
