[cisco-voip] FXO to FXO transfer issue

Ok I have a Cisco 1760 h323 (12.3.11T3) gateway with 8 fxo trucks.
This connects to a cm4.0.

In the instance where a user has their station forwarded to an off
premise number (ie 95551212), and a incoming caller selects their
extension from a Unity auto attendant, the transfer never goes
through.

After entering the extension ie, 111, Unity prompts, Please hold while
I transfer" and puts the caller on hold.

Call manager will xfer. BUT the off premise number will only ring once
then disconnect.

The incoming caller who is supposed to be redirected to the off
premise number, simply remains on hold indefinietly.

Any ideas?
