I had the same problem....
I had to go into the phone and release the IP manually...Could be
holding the reservatoin (I know that's not how DHCP is supposed to work
but doing that worked for me! )
 
The other thing is that have you authorized the DHCP server in Active
directory?
 
I had to run ethereal and do a packet capture to watch the DHCP exchagne
before I saw what the problem was....Could also be a bad NIC on the
server etc etc etc etc

I forgot to say that the DHCP server does show that the phone has
acquired an address.
