[cisco-voip] Call Forward All Issues

Is the forwarded call going out a gateway - same as
the one it came in, another one? Is it a PRI, analog
line, etc? Maybe you can do some debugs and/or look at
call history on gateway to see if the call is even
attempting to go out. Have you looked at a CCM
detailed trace yet?
