[cisco-voip] upgrade assistant 4.1(2)

Bizarre...  There actually WAS 4.1(3) upgrade assistant, and I even
happen to have it ;) It was available in the downloads section under
Callmanager Utilities -
http://www.cisco.com/cgi-bin/tablebuild.pl/callmgr-utilpage. But now there's
only 4.2(1) upgrade assistant there, and the mentioned 4.1(2) under
http://www.cisco.com/cgi-bin/tablebuild.pl/callmgr-41
