[cisco-voip] ccm 4.1.3 busylamp configuration ?

Couple things to check for:

1) Agent phone needs to be configured in callmanager to span voice vlan to
data vlan

2) check ACLs between agent, supervisor, and server networks to see if you
are blocking the RTP stream somewhere along the way

Just from what you said I would definately look at #1
