[cisco-voip] fxo call disconnect problem

Hi all,
I am currently having problem with call not disconnecting on FXO. This FXO
is for outgoing calls only.
c3845-ipvoice-mz.124-5b.bin
Tried a couple of commands from supervisory disconnect though still no luck.

trying now "any tone" now and see what happens.

shut/no shut releases the stucked call.
Can someone give me some advice on how to proceed on troubleshooting this?
