Ok,
I figured this one out.

Standard NI2, B8ZS, ESF.

Two tricky bits: 
1) Make sure that the cisco router is providing clocking and is
configured as the network side.

controller T1 0/0/0
 framing esf
 clock source internal
 linecode b8zs
 cablelength short 133
 pri-group timeslots 1-24 service mgcp 

interface Serial0/0/0:23
 no ip address
 isdn switch-type primary-ni
 isdn protocol-emulate network
 isdn incoming-voice voice
 isdn bind-l3 ccm-manager
 no cdp enable

This brought up the D channel. 

2) But then we were only able to call from Cisco to Nortel.  Calls from
Nortel to Cisco would ring, but when cisco answered the phone, the call
would get dropped and the cisco would hear a fast busy.
Fix:   In the CCMADMIN, under the PRI configuration,
-Check the box "Transmit UTF-8 for Calling Party Name
-Uncheck the box "Display IE Delivery"
-Called party IE number type unknown "unknown"
-called party ie number type unknown "unknown"
Disable IE Presentation bit.

A customer of mine would like to integrate CCM and their existing PBX.
 
I was able to get physical connectivity with a T1 Crossover.  However
I'm having trouble getting layer 2 and the D channel up.
 
On debug isdn q921, I'm sending (TX) the link messages, TX -> SABMEp
sapi = 0 tei = 0

But receiving garbled RX replies. 
RX <- BAD FRAME(0x00017F)
 
The replies are the same as if the line were looped but the PBX guy says
they are not sending me a loop.  

I am providing clock from the router, and the pbx is clock source line.
Isdn switch type on both end:  tried NI2, and DMS-100. (same results)
B8ZS, and ESF (I verified both the PBX and router configs).

Any ideas on where to start troubleshooting this?
