So Call Viewer shows nothing? Did you get an error when you started it up (arbiter error)? If so, are you logging into the unity server with the right account with correct rights/permissions?
In Call Manager, how is the DN set up you have there exactly? Give details at this point.
Are you sure the call is coming into this Call Manager??

Well no luck - it is beyond me why this is so difficult
to have a forwarded call passed to voicemail. If I call 2222222222, everything works as expected, but
when I call 1111111111 - nothing works.
When I look at the Call Viewer in Unity all of those
calls none of the routable fields display any information for those
calls, "Dialled Number" - "Calling Number" - "Forwarding Station" are all
blank. I've tried configuring routing based on DNIS - but again nothing is
being passed to CM, so I assume this is why this also does not work. I've
tried forwarding the call out the PSTN and back (as per Scotts
suggestion), but the same result.
Is there something I can tell the forwarding Telco to
do to correct this perhaps (IE: the equivalent of releasing the call vs.
monitoring the transfer)?

1111111111 is external and not hosted by us or call manager at all.
That telephone number is of another company which is using office space in our
building. In order to use one of our phones (and not have analog lines run
to that office), they simply used a call forwarding service hosted by their
telco to forward to 2222222222.
I will try the static voicemail profile setup again and ensure the device
is reset, and then look into using the call routing in Unity and let you know
how it goes.

This
is regular call manager behavior, it uses the original called party number when
it rolls to voice mail. The VM Profile used would be on the initial DN called,
not VM profile on the forwarded DN since that wouldn't be the original called
number called.
How is the 1111111111
DN setup in Call Manager? Is it a phone, a CTI route point, translation pattern?
Did you set up the VM profile on this number to go the static mailbox number on
unity, and reset the device?
In Unity, you can go under call
routing and set up a rule there to route calls to this number to the unity
mailbox you want. Use the Call Viewer application to see how call comes
into unity.

We are forwarding an external number to a number
hosted by our CM. The call is forwarded as expected and the IP phone
rings, if the call is left unanswered it is transferred to voicemail. The
problem is that the call is not sent to that extensions mailbox as one
would expect; it is transferred to the standard untiy greeting (asking to dial
an extension).
I believe I found the source of the problem but
have been unable to correct the issue. In looking at a CDR record for the
incoming call I have found that the "Called Number" is blank. The
extension is listed as the "Destination Number" and the external number which
has call forwarding enabled is listed as the "Last Redirected
Number".
The extension and voice mailbox are both setup
correctly. If the CM-hosted number is called directly everything functions
as expected, however if the forwarded external number is called the problem
remains. I've tried creating a new voicemail profile which masked the
mailbox to the static mailbox number for that extension.
Ideas?
In the following example CDR dump a person from
(3333333333) called (1111111111) which is forwarded to
(2222222222 (internal extension 2222)).
