Maybe you have the famous "I'll just unplug this machine for I while so that I can plug in my vacuum cleaner, they will never notice..." cleaning-lady syndrome...

 

I have not seen this with Media streaming but i remember back in my mind somewhere that we had a similar problem. The problem was that the switch that the server was connected to died and came back. During this time the network interface went down which led to the fact that the some services could not find the name of the current server. I believe it was CDR insert that stopped for me. This may be completely wrong for your case but maybe worth having a look at.

My dedicated MoH server decided it needed a rest, and we started hearing two beeps when on Hold this morning instead of Lionel Richie

 

While resetting the MoH service is the easy fix, any ideas what happened to break MoH/IP Voice Media Streaming?

I checked the Event Viewer logs and in the early night it did this;

Error: kDBCheckDblGetNodeNameFailed - Database layer select my process node failure.

  Cluster ID: StandAloneCluster

  Node ID: 10.20.29.30

Explanation: The database layer was unable to find/determine the current node on which this service is running.

Recommended Action: Configure the database properly..
