Anything from DNS failure to the HTTP request not able to reach the  
CM.  You can look in IIS Logs to see if the request was hitting the  
server (search on IP address of the phone that had the issue).  If it  
hit the server follow it to tomcat/EM traces (if they were enabled  
and set to detailed).  Otherwise you have to get a sniffer from the  
phone to see why the request wasn't reaching the server.
