I've faced this problem in two customers. In my case, problem was related with a wrong configuration of the MD110 in the PBX side: they were using a sort of signaling not standard that was causing problems. They were not using ISO QSIG.
Hope this helps, I can share the configs of the PBX QSIG trunks if necessary...

This is a problem with path replacement (sort of). Try debugging isdn q.931 and you will se that there are many more ISDN messages than you would expect. One thing you will notice is that the voicemail probably has an "alias" name which is 8890 but in the system itself it may have another number so the MD will give you a redirect to that number. Probably you do not have the route pattern in the Call manager to that number which makes the call fail. The solution will be to set this up as well and make sure that the calling search space for the incoming gateway also contains the route pattern for this outgoing call.

Hi,
I have an issue with CCM where CFwdALL does not work for calls
originating over a Q.SIG trunk to our Ericsson MD110, but does for all
local CCM attached phones.
For example, I set my extension (7702) to forward to voicemail (8890).
Any other VoIP user is correctly transferred to the Voicemail system.
If a call comes in over the Q.SIG trunk (Ericsson user or PSTN user),
the call is not diverted to voicemail, instead the user is given an
engaged signal... It does look like it tries to transfer the call to the
Voicemail system, but fails.
Any suggestions on what could be the cause of this would be very very
helpful, or at least where I could be looking.
The basic setup:
PSTN ----> MD110 ----QSIG----> 1700Rtr ----MGCP---> CCM
