I upgraded to CCM4.0 from 3.0, Cisco 2600 with T1 CAS. I was able to make
call across the T1 CAS both ways on the CCM3.0 MGCP, but not the CCM4.O.
The MGCP shows that is register to the CCM & I can configured the MGCP or
modified the options, but it doesn't make any difference.

this can be archived using COR:
dial-peer cor custom
name internal

assign cor list 'internal' to the 2 phones to be restricted:

assign cor list 'external' to all pots dial-peers used for external calls:

I had the same problems. Microsofts XSDObjectGen together with some
regex-replace and hand coding did the job.
Unfortunately I did this once and now I have to implement new features by
hand - but that's mostly straight forward.
Find attached a VS 2005 C# project which builds a library. Older VS
Versions are possible, the only .NET 2.0 feature used is sgen.exe which
creates an own assembly containing the thousands of serializers in
precompiled form ensures that the first call of a webmethod does not
invoke code generation (if you don't use this, you have to remove the
XmlSerializerAssembly attributes).
