[cisco-voip] Upgrade from CCM3.3 and CRS 3.1 to CCM5.1 and CRS 4.5

Oh, and I found this little ditty:

*Note *if you are using Unified CallManager 5.0 or later, you need to use
CRS 4.5(2) and treat all CRS upgrades as fresh installations.
