[cisco-voip] busy-out the unused B-Channels for IOS MGCP gateway

An MGCP gateway is controlled by CallManager. To busy out the channels on a
Fractional PRI you need to use the "Change B-Channel Maintenance Status"
parameter under Service Parameters - Advanced for the CallManager Service.
This will tell CCM to not send calls to the Busied Channels.

It's not supported, but works if you busy out the B-Channels in IOS.
