[cisco-voip] Meeting Place question

Not sure if this is the right group to discuss this, but has anybody
come across the following behaviour on meeting place:

When participants disconnect from a conference a constant tone appears
and won't clear. This behaviour is random.
Test calls from various systems have been made and the only thing we
have been able to proof so far is that it definitely is absolute random
behaviour..

The meeting place has 3 directly connected trunks into it...
