[cisco-voip] MGCP Gateway Calling Number Manipulation

Unless they've fixed things, if you are in NANPA and you have 10 digit local 
calling then all calls are marked as "NATIONAL" and all your calls will have 
the 91 added to the front, regardless of LD or local.
