There is 1 known pickup fix after your version: CSCsm21224 fixed in 04.2(03)ES54.

Beyond that these are fun to troubleshoot.  have to get CM SDI and SDL traces that cover from the time pickup was working until it stops working. It is usually a bizarre call flow that cause the pickup table leak.  The table exceeds max size and pickup stops working.  Restarting CM is the only recovery.

Phones that are set in the right groups will all of a sudden be not able to pickup calls from phones in the same group. Removing/readding  lines to pickup groups never works, rebooting the CM servers fixes the problem but is never permanent the groups just seem to randomly break.
Has anyone seen this and have any ideas about cause and solution? This was happening when were still on CM 3.3 as well
