Check out the service parameter "Advanced Ad Hoc Conference Enabled".

We notice the Ad hoc conf initator has a * next to their name in  
ConfList. Seems only initiator can add users to conference. I’ll retest.

You have to have 3 parties for a conference.  In the situation you  
describe I'd suspect somebody didn't complete the conference.
