It looks like you need the 'Cisco 3725 -48 V IP Telephony Power Module', 
which is not the Universal DC Power supply option for the router. 

The power supply installation manual at

doesn't list a cisco part # for it though, it sounds like it was
either ordered with or without, depending on if you got the voip
package or not.

Other than optional inline power supply.. 

So, no idea how to order it, even when this router was current. :(
