Do not use voice vlan, try configuring the port only with the VLAN 2, if the IP Phone get the correct IP address, may be there's a problem with the voice vlan configuration.

Is this a 3560 switch? I have seen this behavior several times and have yet to find a solution, the most stressing issue is you put another phone( 7960/40 etc) on that port and it works.

Do you have CDP enabled on the switch?  Cisco phones use CDP voice vlan functionality.

The issue is that a 7912 phone seems to be getting an IP address in the data subnet.  We've tried two configurations on the switchport level and neither are working...

- helper-address on the router
- correct DHCP scope
