[cisco-voip] remove 9 prefix from outbound calls

The issue you are going to run into is your interdigit timeout. If you
have an extension 6143, and someone dials 614-3212, CallManager will
have to wait for the interdigit timeout (T302 timer), or the user will
have to press the # key.

I have callmanager 4.2.3 with cisco 2821 MGCP gatewa, 
in the current senario, users have to dial 9 if they want to call
outside, 
i would like to remove the 9 prefix, and differenciate according to the
number of digits, as the internal extentions are 4 digits numbers, so if
the user dials 4 digits it is inbound, otherwise its outbound.

any ideas?
