[cisco-voip] IPCC Error 13059 and callers struck in talk.

When the agents make a blind transfer to an extension, they get the above error message pop up and eventually get struck in talk. They have to hard reset the phone to get back to the agent desktop. Does anyone can give me some suggestions?
