First of all, that IOS version is horrible. 12.2(3) has lots of VoFR
and DSP fixes you need.

You should; if the upgrade (above) doesn't fix that, contact TAC.

Playout delay increases the jitter buffer, which should help. You're
probably getting lots of jitter from that 64k line, so increasing this
will help mask that.
Make sure your fragment size is set correctly. That should fix most of
your problems.

I wouldn't think it'd be classified as any of these. It all depends on
how your dial plan is set up, however, so I can't say for sure.
