Need wiring pin out for EV-664

I just got a very mint with base EV-664 from a local ham fest. It of course
didn't have the cord or connector. Found the connector at another table
(sound familiar!) but now I     need the pin out for the mic. Does anyone
have this info?

Also is this mic best used for SSB or AM or both?
