Hello All,
Putting this out there just in case anyone needs one.  Don't wanna send to recycling without checking first.
This adapter has a female octal socket on one end and 4 small pins on the other.  Roughly 1.5" in diameter and 1.75" high, including the 4 small pins at the base.  Brown in color with some adhesive tape residue on the outside.  White ink printing is nice and clear.
I am thinking $5 would cover a new box and mailing.  Any extra goes to the cuppa joe fund.
