Bingo...  after receiving this suggesting, called bell and asked them if they
configured it as a Centrex connection, indeed they did and as such it required a
9 prior to calling.  Nice of them to mention this earlier.

Thanks for all the suggestions, at least we eliminated any other possibilities.
