I don't know about internal representation of counters inside IOS
(probably they are two 32-bit words), but there are two ways to
represent 64-bit counter via SNMP:

- 64-bit counters implemented as a single object with the SNMP Counter64
  data type using the SNMPv2c protocol
- 64-bit counters implemented as two 32-bit objects with the SNMP
  Counter (or Counter32) data type using the SNMPv1 and SNMPv2c protocols

Cisco, obviously, uses first approach.
