As a status update to this, we replaced everything.
1. Replaced the MUX. No Change.
2. Replaced the Cables. No Change.
3. Added Ferrite RF shields to the cable. No Change.
4. Replaced the Ciscp PA-MC-T3 card. No Change.
5. Backed down to a Cisco IOS 12.2.19 Service Provider / VIP. No Change.
5. Purchased Attenuators ranging from 2db to 20db. Minor change.
The best we can do currently is the following w/ a 12db Attenuator:

Since we couldn't solve the problem, we moved as many T1s as possible to 8
port cards on a 7206, leaving only 6 T1s on the PA-MC-T3 card. We are
still getting the errors.
I'm really at a loss. Short of putting in a different brand of mux (which
I can do) I'm not sure what else I can do. I have a Carrier Access Wide
Bank 28 and a Telco Systems Edgelink 100 sitting on the shelf.
I'm actually at the point where I am investigating moving to Imagestream
or Juniper gear, as I'm about disgusted w/ Cisco.
