In fact Cisco NAT is "application-aware" to an extent, and does do a certain
amount of inspection inside the packet; see

I know some operating systems and routers communicate with that uPNP stuff,
that automatically opens incoming port mappings based on outgoing traffic.
I did not enable anything like that, and would be surprised if by default
IOS was doing that.........
