[c-nsp] Cisco Customer Satisfaction Survey not working

Has anyone recently managed to complete a Cisco Customer Satisfaction 
Survey?  After closing a number of TAC cases, I would get something like this:

But all I get is an Oracle error of "Deployment ID is not a 
number".  Replying to the email doesn't help either since:

Anyone within Cisco able to ping Bob Ulrich and have him get his act 
together so he is able to get customer feedback? :-)
