It seems that the PCI bridge (dec21150) for this slot is defective.
What is the output of "show pci bridge 5" ?
Do you have the same problem if you reboot the router ? (it could be an
OIR problem).

A PCI ID of 0xFFFFFFFF means "no device". For a DEC21150 you should have
0x00231011.
