Don't mean to turn this into a bug reporter thread but here's another
one worth noting. 

 The "monitor session servicemodule" command is needed because it
enables distributed etherchannel traffic (which requires the central
rewrite engine) to be switched when coming from the FWSM.

Bug ID CSCee10005 discusses connectivity issues with cross-module
etherchannels.
