[c-nsp] sup720 / 6704 RELIABILITY DRIVER / weird ASIC msg

It could be a bug in SXF2.
It could be a bad port asic 0 or bad fabric channel.
It could even be the card is incorrectly seating.

It could also be an incompatible ROMMON version.
12.2(18r)S1 is recommended for SXH.
SXF2 would not recognize this signature as the ROMMON
wasn't released when SXF2 was.

12.2(14r)S5 is probably better for SXF2.

>From what you show the module is running with the
default ROMMON version.
You can determine the ROMMON version with 'show module'

The first thing TACS will tell you is upgrade your IOS version.
SXF2 is ancient and buggy.
