[c-nsp] PA-2FE-TX in 7206VXR

I just picked up a PA-2FE-TX adapter card for use in a 7206VXR router.
Put it in live and the router saw it and just fine:

I added:
If I query interface f6/0, it does see accept it, but not the same for
f6/1 (and again, note missing interface identifier and 0's for the MAC
address):

I have 2 other FastEthernet interfaces in this router as well as an
ATM DS3 - running along without issue - til now with this new
interface card anyway. Total of 4 FastEthernet interfaces - even
though the 'sh ver' above says 3. I have 2 in production use and this
added PA-2FE-TX, for a total of 4.
Anyone seen this behavior before? Tried to get with Cisco on it, but
since all is EOF, they aren't helping.
Any suggestions? Bad interface card maybe?
