On reboot all of the transceivers will fail.
Here is one check to verify the prom controller is working.
You can insert an additional transceiver and see if it is detected correctly.
This isn't definitive since a software problem could also cause it to fail.
But it will give you some idea of the severity of the issue.

Have you had this happen before?

What symptoms (other than the failure of "sh idprom") would you expect? 
How would it affect traffic forwarding (as opposed to operations)?
