I would expect to see it as a directly connected interface but it isn't there. The card is in “slot 1” so I’m thinking that may have something to do with but that’s just a hunch. We also have a 9-port switch in the router as well. “Sh diag” looks clean too. Any ideas?

What does "show interface" display for line protocol?

Try:

interface Serial0/1/0
 encapsulation ppp

 service-module t1 timeslots 1-24 speed 64
 service-module t1 framing esf
 service-module t1 linecode b8zs
