Like I said, I've never tried it, but it looks like you missed a step.

I'm guessing it's 12.2(33)SXI8.  The output from "show version" would
say for sure..

And unrelated to span -

shows there's a 12.2(33)SXI8 and 12.2(33)SXI8a

Usually a rebuild fixes a serious problem.  I'd suggest at least
checking out the release notes for 12.2(33)SXI8a
