For #1, use the Call Forward Busy variable to have each call appearance 
ring to the subsequent lines.  You'll have to put each line in its own 
partition and have the CFB reference a calling search space for that 
partition.  See pg 206 of Cisco Call Manager Fundamentals, "Multiple Call 
Appearances".
