I already have the multiple call appearances. I just want all the lines
to ring. Right now the first call is all that rings. After that the
display shows incoming calls. Am I missing something?

For #1, use the Call Forward Busy variable to have each call appearance
ring to the subsequent lines. You'll have to put each line in its own
partition and have the CFB reference a calling search space for that
partition. See pg 206 of Cisco Call Manager Fundamentals, "Multiple
Call
Appearances".
