[cisco-voip] 7940's Disconnect calls when placed on hold

Is the call using the g.729 codec?  If so, go to the service parameters 
for IP Voice Media Streaming App and make sure that codec is enabled 
for MOH.   I don't remember if this causes disconnects or just no MOH 
but it's worth a shot.
