[cisco-voip] International calling

This is a 5ess PRI running NI2 signalling. Currently everything is set
to CM. With this I get the error message stated in the first email.
This is running MGCP. ISDN and International gets me busy signals.

One problem that I see commonly that seems to match this is when
you're using a 4ESS PRI, but aren't setting the plan/type to
international on the outgoing SETUP. Can you verify switchtype and
post q931 debugs?
