you may want to set up a Linux box and install
asterisk to do your conferencing.
you just need to be able to hand the call off as SIP, or oyu
can also make atserisk work with h.323, but I recommend using SIP
since there's more docs out there for setting it up.
If you have a spare P4 box lying around, you should be able
to set this up fairly easily.
