[cisco-voip] CCM Upgrade from 3.3(3) to 4.1

Lelio, I'm in the same boat, following the same guidance.  Got the CD 
package explicitly so I could go from 3.3(3) to 4.1(2) directly.  Upgrade is 
scheduled for next week -- let me know what you find out.  If need be, I can 
go to 4.0 first, but that seems silly.
