[cisco-voip] Chaining CCM upgrades; 4.0(1) upgrade "failure"

I mentioned a "The memory could not be "read" error" in InsertCDR.exe during 
the 4.0(1) upgrade.  Last night I just clicked OK and gave up in disgust and 
went home to an adult beverage.  When I arrived back this morning the 
"Insert CD 1" prompt was waiting for me.  I did that, and the upgrade 
appeared to complete normally.  I don't see any reference to "InsertCDR.exe" 
in the log.  However, this time I did what I should have done in the first 
place, and Googled InsertCDR.exe.  Two hits pointed to Cisco -- this failure 
is known to occur in 4.0(2) and 3.3(4) upgrades.  Looks like it can happen 
with 4.0(1) as well.  I've seen these things randomly in several machines, 
but all Windows 2000 OSes.  Anyway, it's on to 4.0(2a).
