Or just do it yourself.  The CDR db is very simple.  We had one of our db
guys make an Access interface that brakes all the extension down to
department, multiple by cost and interface with our billing software in
about 3 hours.

CSCdu50004 ability to charge different rates for local/LD/intl calls

CAR is just a basic package, not really scalable.  If you need more billing
features look to Mind CTI or ISI Infotel.
