I don't know how, but it is possible. A service called Vonage does it
all the time (the ATA behind the NAT part, but inbound calling works
fine)

Hello,
Does anyone know how I can configure a ata186 behind a NAT router to
make and receive calls thru a cisco gateway with a public IP address. I
was able to make outgoing calls thru the ata 186 but I have a PRI on the
gateway and I would like to assign one of the DID numbers on the PRI to
the Ata186. The ATA is behind a NAT router and the NAT router has a
dynamic public IP on the internet side. I need a way for the ata to
register on the cisco and sennd some kind of keepalive to indicate when
its IP has changed.
